Quando si parla di nutrizione in oncologia, la prima sensazione che emerge è spesso quella della confusione. Le domande che i pazienti rivolgono più frequentemente sono sempre le stesse: “Cosa posso mangiare?” “Cosa devo evitare?” “Esiste una dieta che aiuta davvero?”
Purtroppo, trovare risposte affidabili non è semplice. Internet è diventato una fonte inesauribile di informazioni, ma anche di disinformazione. Ogni giorno vengono proposte nuove diete, alimenti “miracolosi”, superfood e strategie nutrizionali presentate come risolutive, spesso senza alcun reale fondamento biochimico, biologico o clinico. In molti casi, la visibilità sui social dipende più dalla capacità di comunicare e fare marketing che dalla qualità e robustezza delle evidenze scientifiche.
A questa situazione si aggiunge il peso di oltre settant’anni di convinzioni nutrizionali che continuano a influenzare il modo in cui pensiamo al cibo. Molte raccomandazioni sono nate in un contesto storico completamente diverso da quello attuale, non hanno reali basi scientifiche e vengono ancora ripetute come verità assolute, nonostante oggi conosciamo molto meglio il funzionamento del metabolismo, dell’infiammazione, del microbiota e della biologia del tumore.
Parallelamente si sono diffuse numerose “diete di moda”, spesso sostenute da argomentazioni ideologiche più che scientifiche, contribuendo ad aumentare ulteriormente la confusione.
Esiste inoltre un altro equivoco molto frequente: quello di sovrapporre la nutrizione etica alla nutrizione finalizzata alla salute. Entrambe rappresentano scelte assolutamente legittime, ma rispondono a obiettivi differenti e non dovrebbero essere considerate automaticamente sovrapponibili, soprattutto quando ci si trova ad affrontare una malattia complessa come il cancro.
La conseguenza è che, nella maggior parte dei casi, manca una reale consapevolezza di come il cibo possa influenzare il metabolismo del paziente e il microambiente nel quale cresce il tumore. L’attenzione viene spesso concentrata esclusivamente sulla gestione dei sintomi, attraverso consigli che vengono tramandati quasi automaticamente:
“Se hai nausea, mangia cracker, grissini o biscotti secchi.”
“Se hai la mucosite, mangia gelato o cibi freddi.”
“L’importante è che tu riesca a mangiare qualcosa.”
“Mangia quello che ti senti, mangia anche dolci, l’importante è non perdere peso”.
Si tratta di suggerimenti che possono offrire un beneficio temporaneo sul sintomo, ma che raramente tengono conto delle conseguenze metaboliche dell’alimentazione nel suo complesso sulla malattia oncologica. Ancora oggi, durante molti trattamenti oncologici, ai pazienti vengono spesso offerti tè zuccherati, succhi di frutta, biscotti, cracker, grissini e dolci confezionati: alimenti che possono essere pratici e facilmente tollerati, ma che difficilmente rappresentano la scelta più favorevole dal punto di vista metabolico e infiammatorio.
