When discussing nutrition in oncology, the first impression that often arises is one of confusion. The questions patients ask most frequently are always the same:
“What can I eat?”
“What should I avoid?”
“Is there a diet that can really help?”
Unfortunately, finding reliable answers is far from easy. The internet has become an endless source of information, but also of misinformation. Every day, new diets, “miracle” foods, superfoods, and nutritional strategies are promoted as definitive solutions, often without any genuine biochemical, biological, or clinical foundation. In many cases, visibility on social media depends more on communication and marketing skills than on the quality and strength of the scientific evidence. Adding to this confusion is the legacy of more than seventy years of nutritional beliefs that continue to shape the way we think about food. Many dietary recommendations originated in a historical context that bears little resemblance to today’s scientific understanding. They often lack solid scientific support, yet they continue to be repeated as unquestionable truths, despite the remarkable advances we have made in understanding metabolism, inflammation, the gut microbiota, and cancer biology.
At the same time, numerous “fashionable diets” have gained popularity, often supported more by ideological arguments than by scientific evidence, further increasing the uncertainty. Another common misconception is the tendency to equate ethical nutrition with nutrition aimed at improving health. Both are entirely legitimate choices, but they pursue different goals and should not automatically be considered interchangeable, particularly when dealing with a complex disease such as cancer.
As a result, there is still limited awareness of how profoundly nutrition can influence both the patient’s metabolism and the tumor microenvironment in which cancer develops and progresses. In clinical practice, nutritional advice is often focused almost exclusively on symptom management, relying on recommendations that are passed on almost automatically:
• “If you have nausea, eat crackers or dry biscuits.”
• “If you have oral mucositis, eat ice cream or cold foods.”
• “The important thing is simply to eat something.”
• “Eat whatever you feel like, even sweets, as long as you don’t lose weight.”
These recommendations may provide temporary relief from symptoms, but they rarely take into account the broader metabolic consequences of nutrition on cancer biology. Even today, during many oncological treatments, patients are frequently offered sweetened tea, fruit juices, cookies, crackers, breadsticks, and packaged desserts….
